اثر ساختاری سازمان شخصیت و روابط ابژه بر علائم غیر قابل تبیین از لحاظ پزشکی و دردهای سایکوسوماتیک: نقش واسطه‌ای ذهنی سازی

هدف: این پژوهش با هدف بررسی رابطه ساختاری بین سازمان شخصیت و روابط ابژه بر علائم غیرقابل تبیین پزشکی و دردهای سایکوسوماتیک با توجه به نقش واسطه‌ای ذهنی‌سازی انجام شد. روش‌شناسی: مطالعه حاضر از نوع کمی است که با روش مدل‌سازی معادلات ساختاری به شکل میدانی اجرا گردید. جامعه آماری شامل 450 نفر (18-45 سال) از مراجعه‌کنندگان به مراکز درمانی تهران در سال‌های 1400-1401 بود که به روش نمونه‌گیری در دسترس انتخاب شدند. برای سنجش متغیرها از پنج ابزار استاندارد شامل سیاهه سازمان شخصیت کرنبرگ (IPO)، پرسشنامه روابط ابژه و واقعیت‌آزمایی بل (BORRTI)، ابزار سرند علائم جسمانی (SOMS-2)، پرسشنامه دردهای سایکوسوماتیک (PSQ) و پرسشنامه ذهنی‌سازی (MentS) استفاده شد. تحلیل داده‌ها با نرم‌افزارهای SPSS-23 و AMOS-22 انجام گرفت. یافته‌ها: یافته‌های تحقیق نشان داد که سازمان شخصیت اثر مستقیم مثبت و معناداری بر علائم غیرقابل تبیین پزشکی (β=0.697، p<0.001) و دردهای سایکوسوماتیک (β=0.711، p<0.001) داشت. روابط ابژه نیز اثر مستقیم مثبت و معناداری بر دردهای سایکوسوماتیک (β=0.669، p<0.001) دارد، اما اثر منفی و معناداری هم بر ذهنی‌سازی (β=-0.075، p<0.001) داشت. نتایج دیگر مطالعه نشان داد که ذهنی‌سازی اثر منفی و معناداری بر علائم غیرقابل تبیین پزشکی (β=-0.283، p<0.001) داشت، اما اثر آن بر دردهای سایکوسوماتیک (β=-0.261) معنادار نبود. در پایان نقش واسطه‌ای ذهنی‌سازی در ارتباط بین سازمان شخصیت و علائم غیرقابل تبیین پزشکی و دردهای سایکوسوماتیک، و همچنین بین روابط ابژه و علائم غیرقابل تبیین پزشکی و دردهای سایکوسوماتیک تأیید نشد. شاخص‌های برازش مدل نهایی حاکی از برازش مطلوب با مقادیر (RMSEA=0.11، GFI=0.848، NFI=0.750، RFI=0.893)می باشد. نتیجه‌گیری: برازش مدل ساختاری حاکی از وجود اثر مستقیم و معنادار سازمان شخصیت و روابط ابژه بر علائم غیرقابل تبیین پزشکی و دردهای سایکوسوماتیک است. علاوه بر این، سازمان شخصیت از طریق نقش واسطه‌ای ذهنی‌سازی نیز بر این علائم تأثیرگذار است. این یافته‌ها دارای پیامدهای کاربردی است که می تواند در طراحی و اجرای مداخلات روانشناختی در جمعیت بیماران مورد استفاده قرار گیرد.
