مدلیابی کیفیت زندگی بر اساس استرس ادراک شده و سبک زندگی ارتقا دهنده سلامت با میانجیگری انعطافپذیری شناختی در بیماران مبتلا به بدخیمیهای خونی

هدف: هدف: پژوهش حاضر با هدف مدلیابی ساختاری کیفیت زندگی بیماران مبتلا به بدخیمیهای خونی بر اساس استرس ادراکشده و سبک زندگی ارتقادهنده سلامت با میانجیگری انعطافپذیری شناختی انجام شد. روششناسی: این پژوهش از نوع همبستگی و با رویکرد مدلسازی معادلات ساختاری بود. جامعه آماری شامل بیماران مبتلا به بدخیمیهای خونی کاندید پیوند سلولهای بنیادی خونساز آیت الله بیمارستان طالقانی تهران در بازه زمانی اسفند1403 تا آبان ۱۴۰۴ بود. نمونهگیری به شیوه هدفمند انجام شد و ۲۶۰ بیمار (۱۳۵ مرد و 125 زن) در مطالعه شرکت کردند. ابزارهای پژوهش شامل پرسشنامه کیفیت زندگی سازمان بهداشت جهانی (WHOQOL-BREF)، مقیاس استرس ادراکشده (PSS)، پرسشنامه سبک زندگی ارتقادهنده سلامت (HPLP-II) و پرسشنامه انعطافپذیری شناختی (CFI) بودند. دادهها با استفاده از نرمافزارهای SPSS و Amos و روش مدلسازی معادلات ساختاری تحلیل شدند. یافتهها: نتایج نشان داد استرس ادراکشده تأثیر مستقیم منفی و معنادار بر کیفیت زندگی دارد (β = -0.415, p < 0.001). سبک زندگی ارتقادهنده سلامت قویترین تأثیر مستقیم مثبت (β = 0.517, p < 0.001) و همچنین تأثیر غیرمستقیم مثبت معنادار از طریق میانجیگری انعطافپذیری شناختی (β = 0.131, p = 0.002) بر کیفیت زندگی داشت. انعطافپذیری شناختی نیز تأثیر مستقیم مثبت معنادار بر کیفیت زندگی نشان داد (β = 0.351, p < 0.001)، اما نقش میانجی آن در رابطه استرس ادراکشده و کیفیت زندگی معنادار نبود (β = 0.044, p = 0.245). مدل پیشنهادی برازش مناسبی داشت و ۶۷ درصد از واریانس کیفیت زندگی را تبیین کرد. نتیجهگیری: سبک زندگی ارتقادهنده سلامت و انعطافپذیری شناختی به عنوان عوامل محافظتی کلیدی و استرس ادراکشده به عنوان عامل آسیبپذیری اصلی در کیفیت زندگی بیماران مبتلا به بدخیمیهای خونی عمل میکنند. مداخلات مبتنی بر ترویج رفتارهای سلامتمحور و تقویت انعطافپذیری شناختی همراه با مدیریت استرس میتواند به بهبود کیفیت زندگی این بیماران کمک کند.
