مقایسه اثربخشی مواجهه‌درمانی روایتی و سایکودرام بر افکار منفی مرتبط با تروما در زنان پناهنده افغان: یک مطالعه نیمه‌آزمایشی

چکیده

هدف: پژوهش حاضر با هدف مقایسه اثربخشی مواجهه‌درمانی روایتی و سایکودرام در کاهش افکار منفی مرتبط با تروما در زنان پناهنده افغان انجام شد. روش‌شناسی: این پژوهش نیمه‌آزمایشی با طرح پیش‌آزمون–پس‌آزمون و گروه کنترل انجام شد. جامعه آماری شامل زنان پناهنده افغان ۱۸ سال و بالاتر ساکن تهران در سال 2025 بود. تعداد ۶۰ زن واجد شرایط به روش نمونه‌گیری در دسترس انتخاب و با قرعه‌کشی به‌صورت تصادفی در سه گروه ۲۰ نفری مواجهه‌درمانی روایتی، سایکودرام و کنترل جایگزین شدند. مواجهه‌درمانی روایتی طی ۱۰ جلسه فردی هفتگی ۹۰ تا ۱۲۰ دقیقه‌ای و سایکودرام طی ۱۰ جلسه گروهی هفتگی ۹۰ دقیقه‌ای اجرا شد؛ گروه کنترل مداخله‌ای دریافت نکرد. داده‌ها با استفاده از چک‌لیست رویدادهای زندگی و پرسشنامه شناخت‌های پس از سانحه گردآوری شدند. تحلیل داده‌ها با تحلیل کوواریانس و آزمون تعقیبی بونفرونی در نرم‌افزارهای SPSS نسخه 27 و JASP نسخه 0.19 انجام شد. یافته‌ها: نتایج تحلیل کوواریانس نشان داد که پس از کنترل نمرات پیش‌آزمون، اثر گروه بر افکار منفی مرتبط با تروما معنادار و دارای اندازه اثر بزرگ بود (F=64.324، p<0.001، η²=0.749). بااین‌حال، اثر زمان، تعامل زمان و پیش‌آزمون و تعامل زمان و گروه معنادار نبودند (p>0.05). مقایسه‌های زوجی بونفرونی نشان داد تفاوت مواجهه‌درمانی روایتی با سایکودرام (MD=-4.805، SE=0.575، p<0.001) و گروه کنترل (MD=-6.188، SE=0.566، p<0.001) معنادار بود. همچنین سایکودرام در مقایسه با گروه کنترل کاهش معناداری ایجاد کرد (MD=-1.383، SE=0.554، p=0.049). نتیجه‌گیری: هر دو مداخله در کاهش افکار منفی مرتبط با تروما مؤثر بودند، اما مواجهه‌درمانی روایتی اثربخشی بیشتری نشان داد. بازسازی منسجم روایت زندگی، مواجهه تدریجی با خاطرات آسیب‌زا و بازپردازش معانی مرتبط با تروما می‌تواند این درمان را به گزینه‌ای مناسب برای خدمات روان‌شناختی حساس به فرهنگ در زنان پناهنده افغان تبدیل کند.
