مقایسه توانبخشی شناختی و دارودرمانی بر توجه و حافظه کاری کودکان مبتلا به اختلال کمتوجهی/بیشفعالی

هدف: پژوهش حاضر با هدف مقایسه اثربخشی توانبخشی شناختی و دارودرمانی بر توجه و حافظه کاری کودکان مبتلا به اختلال کمتوجهی/بیشفعالی انجام شد. روششناسی: این پژوهش از نوع نیمهآزمایشی با طرح پیشآزمون–پسآزمون همراه با گروه گواه و مرحله پیگیری بود. جامعه آماری شامل کودکان مبتلا به اختلال کمتوجهی/بیشفعالی شهر ساری بود که با روش نمونهگیری در دسترس انتخاب و بهصورت تصادفی در سه گروه توانبخشی شناختی، دارودرمانی و گواه (هر گروه 15 نفر) جایگزین شدند. ابزارهای پژوهش شامل آزمون استروپ برای سنجش توجه و آزمون انبک برای ارزیابی حافظه کاری بود. گروه توانبخشی شناختی در جلسات تمرینهای ساختاریافته شناختی شرکت کردند و گروه دارودرمانی تحت درمان دارویی رایج قرار گرفتند، در حالی که گروه گواه مداخلهای دریافت نکرد. دادهها با استفاده از تحلیل واریانس چندمتغیری با اندازهگیری مکرر و آزمون تعقیبی بونفرونی در سطح معناداری 05/0 تحلیل شدند. یافتهها: نتایج نشان داد هر دو مداخله توانبخشی شناختی و دارودرمانی موجب بهبود معنادار توجه و حافظه کاری در مرحله پسآزمون نسبت به پیشآزمون شدند (001/0>P). با این حال، میزان بهبود در گروه توانبخشی شناختی بهطور معناداری بیشتر از گروه دارودرمانی و گروه گواه بود. یافتههای تحلیل واریانس اندازهگیری مکرر نشان داد که اثر زمان، اثر گروه و تعامل زمان و گروه برای متغیرهای توجه و حافظه معنادار است. همچنین نتایج آزمون بونفرونی بیانگر تفاوت معنادار بین مراحل پیشآزمون و پسآزمون و نیز پیشآزمون و پیگیری بود، در حالی که بین پسآزمون و پیگیری تفاوت معناداری مشاهده نشد که بیانگر پایداری نسبی اثرات درمان بود. نتیجهگیری: یافتههای پژوهش نشان میدهد که توانبخشی شناختی میتواند رویکردی مؤثر و پایدار برای بهبود توجه و حافظه کاری کودکان مبتلا به اختلال کمتوجهی/بیشفعالی باشد و در برخی شاخصهای شناختی نسبت به دارودرمانی برتری داشته باشد. بر این اساس، استفاده از برنامههای توانبخشی شناختی بهعنوان مداخلهای مستقل یا مکمل در کنار دارودرمانی میتواند در ارتقای عملکرد شناختی، تحصیلی و رفتاری این کودکان مؤثر واقع شود.
